The Health Care Team

Our nurses perform an initial nursing assessment to identify specific needs of our clients. We then work closely with the Physician to create a custom home care plan for each person.

Each nurse is specially trained in adult and/or pediatric health to ensure that the most comprehensive care is given to each client.

Early discharges from acute care can leave people fearful and unsure regarding their care and therapy needs.

Innovative Nursing tries to alleviate these fears through proper education and training of diseases and treatments.

Innovative Nursing also focuses on the ability to promote independence in the home setting and reducing the chance of re-hospitalization.

This reduction is accomplished by continual reinforcement, emotional support and observation of the client and/or caregiver.
